Why metformin sits at this intersection
Metformin was approved for type 2 diabetes in the U.S. in 1994 (used in Europe since 1957). Observational data in diabetic populations show lower all-cause mortality vs. matched non-diabetic controls — a striking signal that motivates longevity-medicine use in non-diabetic adults.
Mechanism
Activates AMPK (cellular energy sensor); inhibits mitochondrial complex I; reduces hepatic gluconeogenesis; modulates gut microbiome. The AMPK activation is mechanistically aligned with caloric restriction.
The TAME trial
TAME (Targeting Aging with Metformin) is the first FDA-approved trial of metformin in non-diabetic adults with aging-related-disease endpoints. The trial design treats aging itself as the intervention target. Results are pending.
Concerns — muscle and exercise
Metformin can blunt the mitochondrial adaptations to exercise (Konopka et al., Aging Cell 2019). Patients prioritizing maximal exercise adaptations may want to discuss timing or alternatives with their physician.
Dosing
Off-label longevity dosing is typically 500–1,500 mg daily, often as extended-release. Most clinicians titrate from 500 mg to manage GI side effects.
