Mechanism — dual 5-alpha reductase inhibition

Dutasteride inhibits both type 1 and type 2 5-alpha reductase isoenzymes, vs. finasteride's type 2 only. The dual inhibition produces ~90% serum DHT suppression vs. finasteride's ~70%. Approved by the FDA for BPH; not approved in the U.S. for AGA (approved in Korea for AGA).

Evidence in AGA

Dutasteride 0.5 mg daily produces greater hair count increase than finasteride 1 mg daily in head-to-head studies (Olsen et al., JAAD 2006). Used off-label, often in patients who plateau on finasteride.

Side effects

Sexual side effects roughly similar to finasteride or slightly higher. Half-life is substantially longer (~5 weeks vs. finasteride's 6–8 hours), meaning side effects (when they occur) clear more slowly. Persistent side-effect syndromes (post-finasteride/post-dutasteride syndrome) remain contested but are reported.

Topical dutasteride

Compounded topical dutasteride 0.1%–0.5% is an emerging approach to capture dual 5-AR inhibition at the scalp with reduced systemic exposure. Evidence base is smaller than topical finasteride but growing.
